| Children birth to 3 years | Referred to Infants & Toddlers Program | Determine need for assessment & types of assessment | Developmental evaluation with the family | Determine if child is eligible for services | If eligible then an Individualized Family Service Plan (IFSP) will be developed |
| Children 3 to 5 years (not enrolled in school) | Referred to Child Find Office | Determine need for assessment & types of assessment | Evaluation by Child Find team | Determine if child is eligible for services | If eligible then an Individualized Education Program (IEP) will be developed |
| School Age Children | Referred to IEP team by Parents or School Staff | Determine need for assessment & types of assessment | Evaluation and review of findings by IEP team | IEP team determines if student is eligible for services | If eligible then the IEP Team will develop an Individualized Education Program (IEP) |
